Troubleshooting the Front USB Panel

  • Phanteks Enthoo Pro ATX full-tower case front panel contains:
    • 2× USB 3.1 ports Not Working
    • 2× USB 2.0 ports Not Working
    • Audio input (mic) 3 mm jack
    • Audio output (headphones) 3 mm jack
    • ATX reset button

Symptoms

  • In Windows,
    • USB mass storage devices do not mount when plugged into any of the USB ports
    • USB Interface devices (mouse, keyboard, etc.) do not work when plugged into any of the USB ports
  • In Linux,
    • USB devices are shown properly in lsusb
    • Mass storage devices are not added to the block devices table (lsblk)
    • Kernel messages (dmesg) shows repeated "reset high-speed usb device number NN using xhci_hcd" and "buffer I/O error on dev" when devices plugged into any front USB port

Actions Taken

  • Move both USB 2.0 and USB 3.0 plugs to alternate USB header pins on motherboard: same symptoms
  • Try known-working USB 2.0 front panel from older machine: same symptoms
  • Re-flash BIOS (already at latest version): same symptoms
  • Clear CMOS: same symptoms
  • Try Enthoo Pro case front panel in different machine with same model motherboard: all ports work as expected
  • Open/Isolated setup: same symptoms
    • Remove motherboard from case and place on non-conductive surface
    • unplug all peripherals except for 1 RAM stick, M.2 SSD, graphics card (no onboard graphics), mouse, keyboard, and front panel USB plugs

Conclusion

Defective USB headers or controller on motherboard
